Evolution of a "Classic" Psychoanalytic Institute: Escape From Rome.

The author describes the evolution of theory and practice at the New York Psychoanalytic Society and Institute. The core concepts at the Institute have included the importance of unconscious fantasy, conflict and compromise theory, transference and countertransference, and defense analysis. In recent years, a variety of contemporary analytic theories have been incorporated into the theory and practice of the Institute. The importance of systematized research to theory development and practice is stressed.

Núcleos neuróticos e não neuróticos / Neurotic and non-neurotic nuclei / Núcleos neuróticos y no neuróticos / Noyaux névrotiques et non névrotiques

Escrito na forma de diálogo com um jovem colega, o artigo tem como ponto de partida material clínico ligado a núcleos neuróticos e não neuróticos em uma mesma paciente. O objetivo é discutir como se constituem esses núcleos, como se dá sua repetição na transferência e como trabalhar com eles na situação analítica.

Written in the form of a dialogue with a young colleague, the article has as its starting point clinical material related to neurotic and non-neurotic nuclei in the same patient. My purpose is to discuss how do these nuclei originate, how their repetition occurs in transference and how to work with them in the analytic situation.

El objetivo es discutir cómo se constituyen esos núcleos, como se da su repetición en la transferencia y cómo trabajar con ellos en la situación analítica.

L'objectif est de discuter la façon dont se constituent ces noyaux, comment leur répétition apparaît dans le transfert et la manière de travailler avec eux dans la situation analytique.

Mutuality.

Ferenczi's striving for mutuality, a call which Freud didn't take up, let him explore this concept with his analysands. He thus became the originator of mutual analysis, although with caveats, and of the concept of introjection, another important Ferenczian notion. The analyst's attitude of knowing the 'objective' and independent Truth is changing its orientation into that of a co-construction in the analytic work; here the analyst and the analysand build a third internal world, which they share and which remains their own. Clinical vignettes illustrate the implications of these views.

Understanding and working with the psychodynamics of practitioner-patient relationships in the manual therapies.

In this paper, we argue that practitioner-patient relationships in the manual therapies would be strengthened by a deeper understanding of the psychodynamics and emotions of those relationships. We suggest that in many cases, a purely bio-mechanical approach may neglect underlying psychological and emotional reasons of the patient's presenting condition, and consequently, lead to a less than adequate outcome for the patient. We offer easily adopted suggestions that could enhance the practice of practitioners of manual therapies as well as other professions that rely on the application of physical methods of diagnosis and treatment. These suggestions could lead to improved prognosis and increased professional satisfaction for practitioners. This paper describes five key dynamics that characterize practitioner-patient relationships: (i) pain as a form of communication; (ii) the 'heart-sink' patient; (iii) dependency; (iv) the erotic transference; (v) endings and loss.

Sandplay therapy with couples within the framework of analytical psychology.

Sandplay therapy with couples is discussed within an analytical framework. Guidelines are proposed as a means of developing this relatively new area within sandplay therapy, and as a platform to open a wider discussion to bring together sandplay therapy and couple therapy. Examples of sand trays created during couple therapy are also presented to illustrate the transformations during the therapeutic process.

Contratransferência e transferência em seminário clínico / Countertransference and transference in a clinical seminar / Contra-transferencia y transferencia en seminario clínico

Este trabalho ilustra, por meio de um caso apresentado em seminário clínico, como o analista disponibiliza sua matéria psíquica viva para dar forma e vida ao objeto primário que um paciente melancólico convocou na/pela transferência. Dessa perspectiva, contratransferência não é apenas a reação emocional do analista, mas principalmente a posição identificatória a partir da qual "alguém" nele se dirige a um aspecto da criança-no-paciente. Sendo inconsciente, esta posição será captada pela escuta analítica nas brechas do relato do caso. Uma exclamação espontânea da analista, altamente carregada do ponto-de-vista emocional, foi o ponto de partida para entrarmos em contato com seu sofrimento narcísico e ambivalência com relação ao paciente. Com a ajuda de uma articulação teórico-clínica em torno da noção da "mãe morta", pudemos reconstruir as vicissitudes não simbolizadas da relação da criança-no-paciente com seu objeto primário. Esse percurso nos levou, por fim, a propor um caminho possível para esta análise.

This paper describes, through a case presented in a clinical seminar, how the analyst makes available his "living psychic material" to give shape and life to the primary object that a melancholic patient summoned in /by transference. From this point of view, countertransference is not only the analyst's emotional reaction but, mainly, the identificatory position from which "someone" in him addresses an aspect of the-child-in-the-patient. Unconscious as such, this position will be captured by analytical listening in the gaps in the narration of the case. A spontaneous exclamation by the analyst, highly charged from an emotional point of view, has been the starting point to get in touch with her narcissistic suffering and ambivalent feelings towards her patient. With the aid of a theoretical-clinical articulation of the notion of the "dead mother", we were able to reconstruct non-symbolized vicissitudes of the relationship between the child-in-the-patient and his primary object. Finally, this trajectory allowed us to propose a way to conduct this analysis.

Este trabajo ilustra, a través de un caso presentado en un seminario clínico, como el analista pone a disposición su materia psíquica viva para darle forma y vida al objeto primario que un paciente melancólico convocó en/por la transferencia. Desde esta perspectiva, la contra transferencia no es apenas la reacción emocional del analista, y sí principalmente, la posición identificadora a partir de la cual "alguien" en él se dirige a un aspecto del niño-en-el-paciente. Siendo inconsciente, esta posición será captada por la escucha analítica en las brechas del relato del caso. Una exclamación espontánea de la analista, altamente cargada desde el punto de vista emocional, fue el punto de partida para que pudiésemos entrar en contacto con su sufrimiento narcisista y su ambivalencia con relación al paciente. Auxiliados por una articulación teórico-clínica en torno de la noción de "madre muerta" pudimos reconstruir las vicisitudes no simbolizadas de la relación del niño-en-el-paciente con su objeto primario. Este trayecto nos llevó, finalmente, a proponer un camino posible para este análisis.
